A request form can also be obtained through the Health Records office via mail, email, or picked up in person. Once completed, the form can be mailed or dropped off at one of the addresses below:

A fee of $30 applies if the file is 20 pages or fewer. Records that are over 20 pages will cost an additional $0.25 per page. 

Please contact the Health Records office if there are barriers to payment. 

The Health Records office is required to respond to the request within 30 days from the date of receipt, or to a maximum of 60 days from the date of the receipt if an extension is required. You will be notified if extended time is required, as well as reasons for the extension. 

If you are a current or former patient, you can review your file in person by visiting the Health Records office located on the campus you received treatment.

Call or email to make an appointment. There is no fee to review your file in person.

If you wish to consent to the release of your personal health records to someone outside your circle of care (such as a lawyer, insurance company, government entity, school, or employer & etc.), you must complete the steps below.

1. Download the Consent to Disclose Form
2. Complete the form: Ensure all required fields are filled out, including:
   • Your full name and date of birth
   • Indicate that you authorize our facility (Royal Ottawa Mental Health Centre or Brockville Mental Health Centre)
   • The recipient’s name and organization (if applicable)
   • The specific information to be disclosed
   • Your signature and your witness’s signature and date
3. Submit the Form by mail, fax, in person or by email. Contact details for the Ottawa and Brockville campuses can be found below.

We follow the Government of Ontario’s Health Information Protection Act as well as our own policies regarding the collection, use and disclosure of information about our patients.
