Once completed, the form and accompanying documentation can be returned to the Health Records department using one of the following methods:

Fees for accessing personal health information will vary depending on the type and quantity of records requested. An estimate will be provided on request.

Please contact the Health Records department if there are barriers to payment.

Fee schedule (effective January 1, 2026)

What do the fee cover?

• Receipt and review of the request 
• Preparation of an invoice estimate, if requested 
• Retrieval of the requested records 
• Preparation and transmission of the requested records (e.g., photocopying, printing, mailing, secure link generation) 
• Communication with the requester 

The Health Records office is required to respond to the request within 30 days from the date of receipt, or to a maximum of 60 days from the date of the receipt if an extension is required. You will be notified if extended time is required, as well as reasons for the extension. 

If you are a current or former patient, you can review your file in person by visiting the Health Records office located on the campus you received treatment.

Download and complete the Consent to Disclose form. This form can be used to consent to the release of your personal health records to someone outside your circle of care (such as a lawyer, insurance company, government entity, school, or employer).
